Aging in Down Syndrome: Morbidity and Mortality

The life expectancy of adults with Down syndrome has increased dramatically over the last 30 years, leading to increasing numbers of adults with Down syndrome now living into middle and old age. Early-onset dementia of the Alzheimer type is highly prevalent in adults with Down syndrome in the sixth decade, and this has overshadowed other important conditions related to aging among adults with Down syndrome. The authors' aim was to update and summarize current knowledge on these conditions, and examine causes of morbidity and mortality in older people with Down syndrome by conducting a systematic review of the published literature for the period: 1993–2008. They reviewed English-language literature drawn from searches in the electronic databases Medline, CINAHL, and PsycINFO, as well as supplementary historical papers. The authors conclude that functional decline in older adults with Down syndrome cannot be assumed to be due only to dementia of the Alzheimer type (which is not inevitable in all adults with Down syndrome). Functional decline may be the result from a range of disorders, especially sensory and musculoskeletal impairments. Given the high rates of early-onset age-related disorders among adults with Down syndrome, programmatic screening, monitoring, and preventive interventions are required to limit secondary disabilities and premature mortality. With respect to assessment and treatment, in the absence of specialist disability physicians, geriatricians have a role to play. *To access this resource, please contact the SHRTN Library Service.*

One of the goals of the Activity and Aging CoP is to educate and advocate on behalf of frail older adults living in care homes for the purpose of establishing realistic, outcome- focused program guidelines that can help provide benchmarks for activity program effectiveness for all homes in Canada. This will be done through the collaborative development of Policy, Procedures and Practice Guidelines for care facilities, specifically for Adult Day Programs and Long-Term Care settings. These recommended guidelines have been brought to you as a direct result of the support and active involvement of key stakeholders - organizations and motivated individuals working in the field of activity programming for the older person. The Activity and Aging Community of Practice would like to extend appreciation to the following organizations and individuals for working with us to ensure these guidelines are practical and meaningful.

Evidence Based Brief - Outcome-based Physical Activity Programming for Seniors in Long-Term Care Homes

Why are outcome-based guidelines for physical activity needed? Scientific evidence suggests that regular physical activity can dramatically influence the health and well-being of people of all ages and abilities (Cress et al., 2005), including the frail elderly living in Ontario’s long-term care homes. Currently in Ontario there are no evidence-based practice guidelines available to support outcome- based physical activity programming for seniors living in long term care homes. Yet, as the population ages and the demands are increased on our long-term care system, physical activity programming that focuses on improving the restorative and functional outcomes of older adults will be critical to maintaining stability in the health system and improving quality of care.

Intact skin – an integrity not to be lost.

Maintaining skin integrity can be challenging but it is vital to overall health, particularly in elderly patients. In this population, skin integrity is frequently compromised as a result of under- or over-hydration, which may cause serious complications. Plans of care must include preventive efforts such as the use of barriers and protectants including zinc oxide preparations, petrolatum- and silicone-based ointments and creams, liquid-forming products, adhesive dressings, fluid managers, skin cleansers, and moisturizers. A team approach that includes the patient, caregivers, and healthcare professionals is needed to address patient concerns regarding independence/dependence, utilization of support systems and services, pain, and control of body fluids. The healthcare provider's role in this team should emphasize continuity of care, patient satisfaction, and product selection - all vital to protecting skin integrity.

Physical activity of Canadian adults: accelerometer results from the 2007 to 2009 Canadian health measures survey

Regular physical activity is associated with a reduced risk of cardiovascular disease, some types of cancer, osteoporosis, diabetes, obesity, high blood pressure, depression, stress and anxiety. As well, strong evidence suggests that higher levels of physical activity are associated with health benefits; in fact, the more activity, the greater the health benefit. To determine whether Canadians are sufficiently active to obtain health benefits, the 2007 to 2009 Canadian Health Measures Survey (CHMS) used accelerometers to collect the first time-sequenced objective measures of physical activity for a nationally representative sample of 6- to 79-year-olds.
