South East (LHIN 10)

In LHIN 10, the Aging with Co-Morbidities (AWCM): Linking Aging, Health and Wellness Network acts as a SHRTN Local Implementation Team (LIT). The AWCM Network is represented by a number of groups and organizations within South East Ontario and was formed in 2008. The LIT function of this Network was adopted in October 2009 through an interest to collaborate with SHRTN.

Mandate, Scope, and Jurisdiction

Within the South East Ontario region, the LIT acts to:

• Put forward suggestions on how to mobilize and achieve SHRTN goals.
• Make recommendations on distribution of resources that support the goals of SHRTN.
• Provide management oversight of local SHRTN activities.
• Provide forum for sharing within SE Ontario of information related to SHRTN activities including Library Service, Knowledge Brokers and Communities of Practice
• Liaise with Local Health Integration Networks.

Meetings

The AWCM group has been meeting monthly since 2008 with meetings regularly scheduled on Wednesdays from 1-4pm.
