

In LHIN 10, the Aging with Co-Morbidities (AWCM): Linking Aging, Health and Wellness Network acts as a SHRTN Local Implementation Team (LIT). The AWCM Network is represented by a number of groups and organizations within South East Ontario and was formed in 2008. The LIT function of this Network was adopted in October 2009 through an interest to collaborate with SHRTN.
