

The Blood Pressure Community of Practice promotes health information aimed at primary care providers to increase cardiovascular health awareness and decrease stroke incidence. This is achieved through a program called the Cardiovascular Health Awareness Program (CHAP).
Since 2006, community care organizations in communities across Ontario have been working together using CHAP to promote cardiovascular health awareness amongst seniors.
In the past year, the CoP has extended its operations in several new communities, building new relationships with individuals and organizations across Ontario. As well, the CoP has made important steps to make links with other CoPs whose mandate is relevant to the prevention and treatment of cardiovascular disease. One area where the CoP has made significant progress is with diabetes educators, and we see opportunity to expand this relationship in the future.
The Blood Pressure CoP membership includes hundreds of paid and unpaid caregivers, researchers, policy makers, and educators. It also includes many Blood Pressure Co-ordinators and management staff in local organizations and family health teams.
