

Hospice Palliative Care is an active compassionate approach that treats, comforts and supports individuals who are living with or dying from a progressive or chronic condition regardless of age, diagnosis, or care setting. Approximately 80% of the palliative care population that is served by these care providers is seniors.
The purpose of the Hospice Palliative Care Community of Practice (CoP) is to improve the quality of care through sharing knowledge and resources to enable evidence-informed care and decision-making. The HPC CoP provides a forum to:
Most recently, the Hospice and Palliative Care CoP hosted an Ontario Telemedicine Network (OTN) presentation by Dr. Larry Librach, “To Err is Human”, completed a province-wide education initiative called Setting the Stage for Change, and an evidence-informed Knowledge Synthesis Report on system design.
The Hospice and Palliative Care CoP connects with 655 persons across Ontario who have expressed interest in participating and / or contributing to CoP and sector events, initiatives or resources.

With the help of SHRTN, the Hospice and Palliative Care CoP plans to create a virtual filing cabinet of palliative care documents that would include system design documents, agendas and minutes for meetings, presentations, policy, and procedures.
