

The SHRTN Diabetes Community of Practice (CoP) brings together diabetes interested professionals, leaders, and direct care providers for the purpose of advancing diabetes care for seniors. Our shared goal is to transform diabetes care for seniors through knowledge exchange, skill building, and creation of competencies to achieve superior outcomes for people living with diabetes.
The diabetes CoP will work collaboratively to address a need for diabetes best practice knowledge dissemination and implementation particularly in the home, community and long term care sectors. The CoP will enhance knowledge synthesis and dissemination of existing diabetes related tools and resources such as the RNAO Best Practice Guidelines, CDA Clinical Practice Guidelines and resources, emerging research and leading practices.

The CoP membership includes a diverse mix of regulated and unregulated care providers, clinical and professional practice leaders, administrators, directors of care, educators, researchers and policy makers. Members of the CoP also include those who serve as partners and enablers in reaching and engaging with current and potential CoP members and participants in CoP activities.
The Core Working Group includes individuals who have committed to assisting the CoP Co-Leads through providing guidance and stewardship, subject expertise, organization, and promotion of CoP activities. This group currently includes representatives from:
