Aging and Developmental Disabilities CoP

The Aging and Developmental Disabilities Community of Practice (ADD CoP) strives to bring together the Developmental Disability, Long Term Care and Community Care sectors in a collaborative exchange to provide seniors with a developmental disability the same access to services as all Ontarians. We have an active core work group of professionals spanning sectors and relevant disciplines and based on our current focus, four project teams driving activities for 2010-12.

Project Teams

U-First Team!: This group provides ongoing knowledge exchange opportunities and support to provincial cross sector U-First trainers, facilitating enhanced program delivery to participants from long-term care, community care and developmental services.

Developmental Disability and Dementia Team: This group facilitates a webinar series providing information and tools to assist caregivers and support for persons with a developmental disability and dementia. The series began with an overview of this topic and is currently moving toward identifying specific knowledge gaps to be addressed going forward.

Cross-Sector Partnership SHOWCASE Team: Facilitating knowledge exchange and support for implementation of a cross sector collaborative project in communities across Ontario. Cross-sector partnership models are shared via the SHOWCASE model; a 6 week, three phase learning process – with a new series delivered every 3 months, which includes opportunities to connect with peers, troubleshoot and support for implementation provided by a knowledge translation tool, online discussions and facilitated networking.

Aging at Home Team: This team supports Aging at Home initiatives that promote cross sector collaboration through the provision of knowledge exchange opportunities to problem-solve, connect with peers and where applicable, collaborate on tool development.

Core Working Group: Membership/Partners

Our core working group is a committed team of CoP members who provide guidance and leadership. The core group members collectively:

  • Commit time and energy to sustain the community’s development
  • Connect members to knowledge and to each other
  • Identify issues and share ideas
  • Disseminate the work of the CoP
  • Serve as social catalysts by making personal connections and relationships
  • Act as stewards of professional knowledge

The Community Networks of Specialized Care (CNSCs) were set up by the Ministry of Community and Social Services across the province of Ontario. The function of these networks is to address the needs of adults (18+) who have a developmental disability, mental health needs and or challenging behaviours.
