SHRTN Funding Application Process 2012-2013

Thank you for your interest in applying to become a part of SHRTN Knowledge Exchange community.

Please note: this is a two-phase deadline process and Phase 1 begins now.

Important Deadlines

The funding application deadlines for 2012-2013 are fast-approaching…

PHASE ONE:December 31, 2011 (for existing and first-time communities & partners)
PHASE TWO: February 15, 2012 (if successful after phase one)

Feel free to sign up for one or both session(s) of your choice. First-time applicants are encouraged to attend at least one of these webinars. Existing CoPs who want a refresher on how Knowledge Exchange and Communities of Practice work as well as review helpful tips are also welcome to attend.

For first-time applicants

If you are a first-time applicant OR want to download the full application package which includes a comprehensive list of supporting documents introducing you to CoPs, Knowledge Brokering and Knowledge to Action (KTA) cycles, you can do so by clicking HERE

You will the option of downloading the application package in the format of your choice (MS Word or PDF).

The application package is comprised of the following documents and forms:

-The application forms (MS Word)
-Terms of Reference
-CoP Orientation Guide.
-Introduction to Knowledge Brokering
